Donation forms

Reusable checkout flows you attach to campaign pages.

Forms are the checkout donors complete: amounts, details, payment, and optional survey. Edit a form once; every linked page updates.

Every live checkout is a named form. Pages never collect through an unnamed built-in widget, so each form can show how much it raised.

Unpublished drafts never appear on live pages.

The first Default form is created from settings (title Default form) if you do not already have one. You can rename it. To use a different form on new pages, open that form and check Default form for new pages. You cannot uncheck it — pick another form instead.

Deleting a form switches its pages to the default form. You cannot delete the last form.

Table columns

Column Meaning
Name Form title in the dashboard and editor.
Layout Accordion (Give monthly / Give once) or Tabs (destination + frequency).
Used on Campaign pages that link this form.
Status Draft or Published.
Raised / Gifts Succeeded donations attributed to this form.
Updated Last save.
Actions Edit, duplicate, delete.

Search by name. Stats above the table count total, published, and draft.

Create a form

  1. Click New form.
  2. Name it.
  3. Pick a template:
    • Standard — accordion frequency, amounts, checkout, follow-up.
    • Tabs — destination picker, tab frequency, local gift routing.
  4. Open the form editor.

New forms also copy Donation form defaults where those apply.
